Licensed Case Manager QMHP -CBCC (Full-Time)

A member of a multi-disciplinary team responsible for ensuring members are provided with case management and Dual Enhanced Outpatient Treatment services. CBCC is funded by the State of Hawaii, Department of Health, Med Quest Division, in collaboration with Kaiser Permanente, Aloha Care, Ohana Health Plan, United Health Care and Adult Mental Health Division. Community Care Services (CCS) is a funding source of Ohana Health Plan. CBCC/CCS and Dual Enhanced Outpatient Treatment services are voluntary programs that enhance the consumer’s strengths rather than their mental illness, symptoms, and psychiatric history. The program will rely on the talents, skills, and abilities of all its staff members in order to function effectively, and achieve the program goals. The program under the supervision of the Helping Hands Hawaii (HHH), Medical Director, QMHP and Program Director and Team Leader, to ensure adequate staff to support a safe, structured, and therapeutic environment in which members and staff work. The CBCC/CCS staff member is to comply with the company code of ethics, confidentiality practices, HIPAA, safety and all other applicable policies and procedures.



  • 2 years of specialized experience, An individual who is independently licensed as: A licensed psychologist; A certified in clinical social work with an master’s degree in social work and is licensed (LCSW);
  • A registered nurse who is certified by the American Nurses Credentialing Center as a psychiatric nurse or mental health clinical nurse specialist, or has a master’s degree in nursing with a specialization in psychiatric/mental health and is licensed to practice as a psychiatric or mental health nurse (APRN); An licensed professional counselor who is licensed clinical practitioner (LMHC, LMFT, LAC)
  • Must have strong commitment to the right and ability of each person with severe and persistent mental illness to live in normal community residences; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services,
  • Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for client rights and personal preferences in treatment are essential,
  • Valid Hawaii Driver’s license and willingness to use properly insured car, Clear pre-employment reference check, clear driver’s abstract, properly insured vehicle, annual TB Clearance, CPR, First Aid and CPI certification.
