|A member of a multi-disciplinary team responsible for ensuring members are provided with case management and Dual Enhanced Outpatient Treatment services. CBCC is funded by the State of Hawaii, Department of Health, Med Quest Division, in collaboration with Kaiser Permanente, Aloha Care, Ohana Health Plan, United Health Care and Adult Mental Health Division. Community Care Services (CCS) is a funding source of Ohana Health Plan. CBCC/CCS and Dual Enhanced Outpatient Treatment services are voluntary programs that enhance the consumer’s strengths rather than their mental illness, symptoms, and psychiatric history. The program will rely on the talents, skills, and abilities of all its staff members in order to function effectively, and achieve the program goals. The program under the supervision of the Helping Hands Hawaii (HHH), Medical Director, QMHP and Program Director and Team Leader, to ensure adequate staff to support a safe, structured, and therapeutic environment in which members and staff work. The CBCC/CCS staff member is to comply with the company code of ethics, confidentiality practices, HIPAA, safety and all other applicable policies and procedures.
